CHP will prioritize and expand housing stability opportunities as a fundamental social determinant of health.
How we’ll do it
- Construct and/or preserve affordable rental and single-family housing in areas that both demonstrate the need for affordable housing and have the financial and community support that will make a project successful.
- Sustain Housing Stability as a key focus area with collaborative outreach, engagement, and assistance to help CHP residents maintain stable rental housing or make a transition to homeownership.
- Implement strategies to expand weatherization work beyond energy efficiency upgrades for a more holistic approach to creating healthier homes.
- Prioritize services to seniors in response to an aging national population dealing with chronic health conditions so that more seniors can successfully age in place.
CHP will build partnerships within the healthcare industry that result in capital investment in CHP’s affordable housing development and service delivery work.
How we’ll do it
- Senior leadership will participate in NeighborWorks America Health Partnership Investment (HPI) Learning Lab opportunity to build capacity and knowledge.
- Leverage existing healthcare partnership models, to include the J. Van Story Branch Health & Wellness Suite, to attract new healthcare partnership and investment opportunities.
CHP will implement processes for the inclusion of resident, client, and community voice in decision making and program design.
How we’ll do it
- Deploy an annual resident survey across all of CHP’s owned and managed rental communities.
- Create and deploy ad hoc resident or client surveying to gather resident voice around specific challenges or issues within a community.
- Perform community surveying/engagement activities to inform rental and single-family development projects.
